Read and print out the "Terms and Conditions Form"  . |

Please read and sign the Terms and Conditions Form after printing. |

Then either Email or FAX a copy to Sea Sports Travel at 281-477-7395. |

Be sure to bring a copy with you to the boat on the day of your departure. |
